THY u\u00e7a\u011f\u0131nda pilotaj bir hata olmad\u0131\u011f\u0131 kaydedilen  raporda, Schiphol Havaalan\u0131 da y\u00fczde 10 su\u00e7lu bulundu. THY y\u00f6netim  kurulu rapora ili\u015fkin yapt\u0131\u011f\u0131 toplant\u0131n\u0131n ard\u0131ndan g\u00f6r\u00fc\u015flerini kamuoyu  ile payla\u015ft\u0131.<\/a><\/p>\n<p><a> Kazaya ili\u015fkin Hollanda Ulusal G\u00fcvenlik Konseyi Ara\u015ft\u0131rma  Komisyonu&#8217;nun kesinle\u015fmi\u015f raporu Lahey kentinde bug\u00fcn a\u00e7\u0131kland\u0131.  Yakla\u015f\u0131k 5 ay \u00f6nce a\u00e7\u0131klanan raporla benzerlik ta\u015f\u0131yan nihai raporda,  kazaya irtifa g\u00f6stergesindeki bozuklu\u011fun neden oldu\u011fu ve kazadan y\u00fczde  80 oran\u0131nda Boeing \u015firketinin sorumlu oldu\u011fu vurguland\u0131. \u0130stanbul&#8217;dan  124 yolcu ve 7 m\u00fcrettebatla Amsterdam&#8217;a giden THY u\u00e7a\u011f\u0131n\u0131n  Schiphol  Havaalan\u0131&#8217;na ini\u015fte d\u00fc\u015fmesinde pilotaj hatas\u0131 olmad\u0131\u011f\u0131 ifade edilen  raporda, kaza nedeni, Boeing 737 tipi u\u00e7a\u011f\u0131n y\u00fckseklik \u00f6l\u00e7en altimetre  g\u00f6stergesinin sebep oldu\u011fu belirtildi.<\/a><\/p>\n<p><a> &#8220;THY Y\u00d6NET\u0130M\u0130 RAPORU DE\u011eERLEND\u0130RD\u0130&#8221;<\/a><\/p>\n<p><a> THY y\u00f6netim kurulu da toplant\u0131n\u0131n ard\u0131ndan TK1951 \u0130stanbul-Amsterdam  seferini yapmakta iken ini\u015f s\u0131ras\u0131nda kaza ge\u00e7iren TC-JGE tescilli  u\u00e7akla ilgili haz\u0131rlanan raporu de\u011ferlendiren bir a\u00e7\u0131klama yapt\u0131.  \u00d6ncelikle Hollanda Emniyet Kurulu (Dutch Safety Board) taraf\u0131ndan  yay\u0131nlanan bu t\u00fcr raporlar\u0131n amac\u0131n\u0131n, olay\u0131n meydana geli\u015f bi\u00e7imini ve  tekrarlanmamas\u0131 i\u00e7in neler yap\u0131lmas\u0131 gerekti\u011fini ara\u015ft\u0131rmak i\u00e7in  haz\u0131rland\u0131\u011f\u0131na i\u015faret edilerek,  bug\u00fcn yay\u0131nlanan s\u00f6z konusu raporun da  bu anlay\u0131\u015fla incelendi\u011fi belirtildi.<\/a><\/p>\n<p><a>THY y\u00f6netimi, olay\u0131n olu\u015f \u015fekline ili\u015fkin izah yapt\u0131ktan sonra rapora  ili\u015fkin g\u00f6r\u00fc\u015flerini maddeler halinde  kamuoyuna aktard\u0131.<\/a><\/p>\n<p><a> Buna g\u00f6re, B737-800 u\u00e7aklar\u0131nda yer ile olan d\u00fc\u015fey mesafeyi \u00f6l\u00e7en  iki radyo altimetre sistemi mevcut oldu\u011fu hat\u0131rlat\u0131larak, \u015fu bilgiler  verildi; &#8220;TC-JGE u\u00e7a\u011f\u0131n\u0131n, Amsterdam Schiphol Meydan\u0131 18 sa\u011f pistine  yakla\u015fmas\u0131 s\u0131ras\u0131nda, o anda u\u00e7a\u011fa kumanda etmekte olan 2. pilotun  kulland\u0131\u011f\u0131 sa\u011f radyo altimetre normal \u00e7al\u0131\u015f\u0131rken, soldaki -8ft de\u011ferinde  hatal\u0131 bir y\u00fckseklik bilgisi vermi\u015ftir. Otomatik u\u00e7u\u015f sisteminin h\u0131z\u0131  kontrol eden k\u0131sm\u0131 olan otomatik gaz sistemi (autothrottle), tasar\u0131m  \u00f6zelli\u011fi olarak yere yak\u0131nl\u0131k bilgisini sa\u011f radyo altimetre de\u011feri ile  kar\u015f\u0131la\u015ft\u0131rmadan sadece sol radyo altimetreden ald\u0131\u011f\u0131 i\u00e7in, gelen hatal\u0131  bilgiyi u\u00e7a\u011f\u0131n yerle temas an\u0131nda oldu\u011funun bir g\u00f6stergesi olarak  de\u011ferlendirmi\u015ftir. Otomatik gaz sistemi bunun sonucu olarak gaz\u0131 keserek  h\u0131z\u0131n kontrol edilebilir en d\u00fc\u015f\u00fck seviyenin (stall h\u0131z\u0131) alt\u0131na  d\u00fc\u015fmesine ve yere \u00e7ok yak\u0131n bir y\u00fckseklikte u\u00e7a\u011f\u0131n kontrol\u00fcn\u00fcn  kaybedilmesine neden olmu\u015ftur. Otomatik gaz sistemi ile sol radyo  altimetre aras\u0131ndaki hataya yatk\u0131n bu ili\u015fkiye u\u00e7ak \u00fcreticisi taraf\u0131ndan  pilotlar i\u00e7in haz\u0131rlanan dok\u00fcmanlarda kaza \u00f6ncesi yer verilmemi\u015fken,  kaza sonras\u0131 yay\u0131n yap\u0131larak dok\u00fcmanlara konulmu\u015ftur.&#8221; U\u00e7a\u011f\u0131n, Amsterdam  Hava Trafik Kontrol\u00fc taraf\u0131ndan normalden daha k\u0131sa bir yakla\u015fma  rotas\u0131yla getirildi\u011finden, olmas\u0131 gerekenden y\u00fcksek ve h\u0131zl\u0131 kald\u0131\u011f\u0131n\u0131  belirten THY, u\u00e7a\u011f\u0131n gaz kollar\u0131n\u0131n y\u00fcksek h\u0131z\u0131 d\u00fc\u015f\u00fcrme amac\u0131yla  r\u00f6lantide olmas\u0131 gerekti\u011fi beklentisinin, ola\u011fan d\u0131\u015f\u0131l\u0131k alg\u0131lanmas\u0131n\u0131  \u00f6nledi\u011fini ifade etti.<\/a><\/p>\n<p><a> &#8220;BOE\u0130NG&#8217;\u0130N P\u0130LOTLAR \u0130\u00c7\u0130N HAZIRLADI\u011eI DOK\u00dcMANDA B\u0130LG\u0130LER EKS\u0130K&#8221;<\/a><\/p>\n<p><a> H\u0131z, kontrol edilebilir de\u011fere yakla\u015ft\u0131\u011f\u0131nda, g\u00f6rsel ve i\u015fitsel ikaz  sistemlerinin devreye girdi\u011fi ve hemen ard\u0131ndan kurtarma manevras\u0131na  ba\u015fland\u0131\u011f\u0131 izah edilerek, kaza \u00f6ncesi ya\u015fananlar \u015fu \u015fekilde anlat\u0131ld\u0131;  &#8220;Ancak bu manevra s\u0131ras\u0131nda kokpit ekibi taraf\u0131ndan a\u00e7\u0131lan gaz kollar\u0131  hatal\u0131 altimetre bilgisi nedeniyle otomatik gaz sistemi taraf\u0131ndan geri  \u00e7ekilmi\u015f, hemen ard\u0131ndan sistem ekip taraf\u0131ndan devreden \u00e7\u0131kar\u0131larak gaz  kollar\u0131 yeniden a\u00e7\u0131lm\u0131\u015ft\u0131r. Bu durumdan kaynaklanan zaman kayb\u0131 ve  d\u00fc\u015fey mesafenin yetersizli\u011fi nedeniyle kurtarma manevras\u0131 ba\u015far\u0131s\u0131z  olmu\u015ftur. U\u00e7ak \u00fcreticisinin dok\u00fcmanlar\u0131nda b\u00f6yle bir durumda pilotlar\u0131n  derhal gaz kollar\u0131n\u0131 a\u00e7malar\u0131 gerekti\u011fi belirtilmesine kar\u015f\u0131n, otomatik  gaz sisteminin devreden \u00e7\u0131kar\u0131lmas\u0131 gere\u011fine de\u011finilmemi\u015ftir.&#8221;<\/a><\/p>\n<p><a> Hollanda Emniyet Kurulu&#8217;nun a\u00e7\u0131klad\u0131\u011f\u0131 raporda yap\u0131lan tespitlerden  baz\u0131lar\u0131 ise maddeler halinde \u015f\u00f6yle s\u0131raland\u0131:&#8221;<\/a><\/p>\n<p><a> 1 &#8211; THY Teknik A.\u015e. B737-800 u\u00e7aklar\u0131nda yanl\u0131\u015f bilgi veren radyo  altimetre problemini kazadan uzun s\u00fcre \u00f6nce fark etmi\u015f ve bu konuda   yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmalar\u0131n yan\u0131 s\u0131ra gerek u\u00e7ak \u00fcreticisi  gerekse anten  \u00fcreticisi nezdinde bildirimlerde bulunmu\u015ftur. THY Teknik A.\u015e., ayn\u0131  zamanda sorunu u\u00e7ak \u00fcreticisinin resmi internet portal\u0131nda g\u00fcndeme  getirerek tart\u0131\u015f\u0131lmas\u0131n\u0131 ve di\u011fer bir\u00e7ok B737-800 kullan\u0131c\u0131s\u0131n\u0131n benzer  radyo altimetre problemlerini aktarmalar\u0131n\u0131 sa\u011flam\u0131\u015ft\u0131r. Ancak bu  sorunlar \u00e7\u00f6z\u00fclmedi\u011fi halde konu bir s\u00fcre sonra \u00fcretici taraf\u0131ndan  g\u00fcndemden \u00e7\u0131kart\u0131lm\u0131\u015ft\u0131r.<\/a><\/p>\n<p><a> 2 &#8211; Raporda u\u00e7ak \u00fcreticisinin \u00e7ok say\u0131da B737-800 u\u00e7a\u011f\u0131nda g\u00f6zlenen  bu durumu ciddi bir u\u00e7u\u015f emniyet sorunu olarak g\u00f6rmedi\u011fi belirtilmi\u015ftir.  Ancak 2006 y\u0131l\u0131 sonras\u0131 \u00fcretilen B737-800 u\u00e7aklar\u0131n\u0131n, fabrika  \u00e7\u0131k\u0131\u015f\u0131nda birinci ve ikinci radyo altimetre de\u011ferlerini k\u0131yaslayan ve  fark olmas\u0131 durumunda u\u00e7u\u015f ekibini uyaran bir sistemle donat\u0131lm\u0131\u015f olmas\u0131  dikkat \u00e7ekicidir. Raporda, u\u00e7ak \u00fcreticisinin ve u\u00e7a\u011fa sertifikasyon  sa\u011flayan Amerikan Sivil Havac\u0131l\u0131k Otoritesinin, TC-JGE dahil olmak \u00fczere  bir\u00e7ok B737-800 u\u00e7a\u011f\u0131nda hatal\u0131 altimetre bilgilerine sebep olan bu  tasar\u0131m sorununa yeterince \u00f6nem vermemi\u015f oldu\u011fu belirtilmi\u015ftir. <\/a><\/p>\n<p><a> 3- Hollanda Hava Trafik Kurallar\u0131na g\u00f6re, Amsterdam Schiphol Meydan\u0131  18 sa\u011f pistine aletli yakla\u015fma uygulayan bir u\u00e7ak pist do\u011frultusuna,  pist ba\u015f\u0131na g\u00f6re en az 8 deniz mili mesafede ve 2000ft  y\u00fckseklikte  konumland\u0131r\u0131lmal\u0131d\u0131r. Ancak buna ra\u011fmen hava trafik  kontrol\u00f6rleri,  trafik ak\u0131\u015f\u0131n\u0131 h\u0131zland\u0131rmak  amac\u0131yla kendi inisiyatifleriyle, pist  ba\u015f\u0131na 5 deniz mili mesafeye kadar konumlama yapabilmektedir. Raporda,  bu uygulaman\u0131n u\u00e7aklar\u0131n y\u00fcksek ve h\u0131zl\u0131 kalmalar\u0131na neden olarak u\u00e7u\u015f  ekiplerinin son yakla\u015fma evresindeki i\u015f y\u00fck\u00fcn\u00fc artt\u0131rd\u0131\u011f\u0131 belirtilmi\u015f,  ba\u011fl\u0131 olarak hava trafik kontrol\u00f6rlerinin bu uygulamas\u0131n\u0131n emniyetli  olmad\u0131\u011f\u0131 de\u011ferlendirilmi\u015ftir.<\/a><\/p>\n<p><a> 4 &#8211; Bu hava trafik kontrol uygulamas\u0131, u\u00e7a\u011f\u0131n toplam enerjisinin  pist ba\u015f\u0131na olan mesafeyle ba\u011flant\u0131l\u0131 olarak gerekenden daha fazla  olmas\u0131na neden olmu\u015f, otomatik gaz sistemi mevcut h\u0131z\u0131 son yakla\u015fma  h\u0131z\u0131na d\u00fc\u015f\u00fcrebilmek i\u00e7in gaz kollar\u0131n\u0131 r\u00f6lantide tutmu\u015ftur. Pilot  taraf\u0131ndan otomatik u\u00e7u\u015f sistemine do\u011fru olarak girilmi\u015f ini\u015f h\u0131z\u0131na  ula\u015f\u0131ld\u0131\u011f\u0131nda, bu kez hatal\u0131 altimetre bilgisi otomatik gaz sistemini  yan\u0131ltarak gazlar\u0131 a\u00e7mas\u0131n\u0131 engellemi\u015f ve h\u0131z seri olarak d\u00fc\u015fmeye  ba\u015flam\u0131\u015ft\u0131r.<\/a><\/p>\n<p><a> Yakla\u015f\u0131k 460ft y\u00fckseklikte, u\u00e7aktaki g\u00f6rsel ve i\u015fitsel ikaz  sistemleri u\u00e7a\u011f\u0131n havada tutunamaz duruma (stall) girmekte oldu\u011fu  ikaz\u0131n\u0131 vermi\u015f, bunun \u00fczerine u\u00e7u\u015f ekibi derhal kurtarma manevras\u0131na  ba\u015flam\u0131\u015ft\u0131r. Bu manevran\u0131n bir gere\u011fi olarak gaz kollar\u0131 ileri itilmi\u015f  ve l\u00f6vye yard\u0131m\u0131yla bu durumdan \u00e7\u0131k\u0131lmaya \u00e7al\u0131\u015f\u0131lm\u0131\u015ft\u0131r. Ancak otomatik  gaz sistemi hatal\u0131 altimetre bilgisini kullanmaya devam ederek gaz  kollar\u0131n\u0131 yeniden r\u00f6lantiye \u00e7ekmi\u015ftir. Durumu fark eden u\u00e7u\u015f ekibi,  sistemi devre d\u0131\u015f\u0131 b\u0131rakarak gaz kollar\u0131n\u0131 tekrar a\u00e7m\u0131\u015ft\u0131r. Rapor, u\u00e7ak  \u00fcreticisinin acil durum uygulamalar\u0131n\u0131 i\u00e7eren dok\u00fcman\u0131nda, stall&#8217;dan  kurtulmak i\u00e7in otomatik gaz sisteminin devre d\u0131\u015f\u0131 b\u0131rak\u0131lmas\u0131  gerekti\u011fine dair bir maddenin olmad\u0131\u011f\u0131n\u0131 belirtmektedir.<\/a><\/p>\n<p><a> 5 &#8211; Rapora g\u00f6re sim\u00fclat\u00f6rde yap\u0131lan testler, B737-800 u\u00e7a\u011f\u0131n\u0131n  stall&#8217;dan kurtulmas\u0131 i\u00e7in en az 500ft seviye gerekti\u011fini g\u00f6stermi\u015ftir.  U\u00e7a\u011f\u0131n yere g\u00f6re yakla\u015f\u0131k 460ft seviyede stall uyar\u0131s\u0131 verdi\u011fi g\u00f6z\u00f6n\u00fcne  al\u0131nd\u0131\u011f\u0131nda, yukar\u0131daki de\u011ferlendirmeye g\u00f6re kurtulma olana\u011f\u0131 olmad\u0131\u011f\u0131  a\u00e7\u0131kt\u0131r.T\u00fcrk Hava Yollar\u0131, bu kaza ara\u015ft\u0131rmas\u0131ndan edinilen bilgiler  \u0131\u015f\u0131\u011f\u0131nda, kaza sonras\u0131 ivedi olarak b\u00fct\u00fcn B737-800 pilotlar\u0131n\u0131, hataya  yol a\u00e7abilecek tasar\u0131m\u0131n sonucu olarak yanl\u0131\u015f radyo altimetre bilgisinin  otomatik gaz sistemi taraf\u0131ndan kullan\u0131labilece\u011fi konusunda  bilgilendirmi\u015f ve d\u00fc\u015f\u00fck irtifada stall&#8217;dan kurtulma manevras\u0131 konusunda  sim\u00fclat\u00f6rde e\u011fitmi\u015ftir.&#8221;<\/a><\/p>\n<p><a> &#8220;RAPORA KAR\u015eI TESP\u0130TLER DE \u0130FADE ED\u0130LD\u0130&#8221;<\/a><\/p>\n<p><a> THY ortakl\u0131\u011f\u0131, raporda hemfikir olmad\u0131\u011f\u0131 tespitler bulundu\u011funu  belirterek, en \u00f6nemli iki tanesi, &#8220;Yakla\u015fman\u0131n istikrarl\u0131 olup olmamas\u0131  kazan\u0131n olu\u015fumunda etken de\u011fildir. Halbuki, ekibin an\u0131nda  m\u00fcdahalesine ra\u011fmen, otomatik gaz sistemi gaz kollar\u0131n\u0131 beklenmedik bir  \u015fekilde geriye \u00e7ekmi\u015ftir. Otomatik gaz sistemini devreden \u00e7\u0131karan ekip,  gaz\u0131 ikinci kez a\u00e7m\u0131\u015f, ancak yetersiz y\u00fckseklik nedeniyle kaza  \u00f6nlenememi\u015ftir.
